For an eye examination for a new patient, or if it
has been greater than 2 years since your last visit
to our office, please print and fill out Forms 1 and 2
and 3. If you are interested in contact lenses,
please print and sign Form 4.

The Privacy Form consists of 3 pages. You only need to
sign and bring in the final page. The other two pages are
for your information only.

If you will print these Forms out and bring them to your
appointment, you will save a considerable amount of time.
